MAKING A PALLIATIVE CARE REFERRAL

Anyone can make a referral. Whether a patient, a carer, a friend or family member, but we must have the patient’s full consent before we proceed with a palliative care referral.


We encourage you to make a referral TO PALLIATIVE CARE as soon after diagnosis as possible.

This is because we can support a meaningful improvement in people’s ability to cope with challenging circumstances; so the earlier someone is referred to hospice, the more chance we have to help.

Who do we care for?

Our palliative care services are offered to people over the age of 18 with a life-limiting illness and hospice care needs. We can care for patients with a wide range of needs.

How much does it cost?

There’s no charge and nobody will mention money to you or your family. If you would like to donate, you are of course very welcome but you are under no obligation to do so.

Opening times

We are currently open for Day Hospice on Wednesdays from 10.30 am til 4 pm. We are working toward being open 24/7 all year round, but for now, we offer planned Respite for up to seven days each month.

What is the catchment area?

There’s no geographical restriction, but most of our patients come from the three counties of Carmarthenshire, Ceredigion and Pembrokeshire as travelling from further afield can be difficult when people are feeling unwell.

Who provides clinical care?

We have highly skilled nurses providing clinical care during respite and a wealth of experience across the care team. There is no formal handover of care when you come to stay with us, so your own specialist medical teams remain the lead clinicians.

Do I have to be religious?

Absolutely not. We are a diverse organisation and seek to respect everyone as they are, regardless of age, gender, culture or belief. It is extremely important to us that all are made to feel equally welcome.
